I comply with the ICO (Information Commissioner's Office) which means I need to tell you what data I am collecting from you and what I intend to do with it.

What data do I keep and why do I need it?

Name and age – this is basic information that helps me get to know you

Address, email address, phone number – I use this as a way of contacting you regarding your sessions. I will mainly use email or text but, if I can’t reach you, I will try a different method.  I may also contact you about opportunities that may be helpful to you, such as groups, courses, experiences, voluntary work, etc.

Doctors details – If I am worried that you are at risk then I may need to contact your doctor.  If I can, I will tell you I am going to do this.

Assessment and Session notes – I keep brief anonymous notes of your sessions.

Will I share your data and, if I do, who will I share it with and for what purpose?

It is very unlikely that I will share your data. I will not sell it on or use it for unethical reasons. I may have to share it if notes are subpoenaed by court; if you or anyone you tell me about is at harm or risk of harm, I may have to pass this information on to your GP or the emergency services. I also have regular supervision where I talk about my work but I only use your first name.

How will we store your data?

Paper based data is kept in a locked filing cabinet. I transfer your name and a code to a password protected laptop and your phone numbers may be kept on a mobile phone which is code protected.

How long will I store your data for and how will I dispose of it?

I will keep your session notes for 8 years after the end of our sessions as required by my governing body. (If you are under 18, I am required to keep your notes until you are 25 years old.)

I will delete your phone number out of my mobile phones as requested after our work finishes.

Consent

By using the contact form on the website you are giving consent for me to use your data in this way.   If we agree to work together, you will be asked to sign this agreement at the start of therapy.
